A privately held firm that certifies you as qualified to run ads. That is all there is to it, yet most explanations overlook what actually matters for your business by describing LegitScript as a certification body. For a telehealth operator, what it truly amounts to is the barrier separating your clinic from any paid traffic whatsoever.

A US telemedicine provider cannot advertise on Google without LegitScript certification, and Google's healthcare advertising policy explicitly names the program. Microsoft, Meta, LinkedIn and TikTok hold the same stance, and the majority of payment processors require it for underwriting.

So a private company based in Portland, Oregon determines who may advertise and who may accept card payments in this sector, and no one voted them into that role.

This also explains why the LegitScript application belongs in week one of ClinicBuilder's launch protocol instead of somewhere near the finish. Nothing else in a clinic can begin generating revenue until it gets through.

What they actually do

LegitScript verifies that a merchant operating in a regulated space complies with the rules that apply to it, and then continues to monitor them.

The certification is a seal, and behind that seal sits a folder. State licenses, entity registrations, DEA certificates where prescribing involves controlled substances, a privacy policy, pharmacy relationships and marketing claims.

The platforms have handed their due diligence over to a contractor. Determining whether a telehealth clinic is a genuine medical practice or a pill mill requires expertise Google lacks and has no interest in developing, so it pays another party to keep the file.

The three certification programs

Healthcare Certification serves telemedicine providers, online pharmacies and related merchants. This is the one that gets you approved on Google, Microsoft, Meta, LinkedIn and TikTok.

Addiction Treatment Certification serves treatment providers and gets you approved on Google, Microsoft and Meta.

CBD Certification covers Google, Microsoft and Facebook.

A tier designed for large companies exists at the enterprise level. If you are starting a clinic, Healthcare Certification is the one you need, and it is also what ClinicBuilder applies for on the clinics it constructs.

Is LegitScript legitimate

That is a reasonable question, since a company you had not even heard of a week ago is asking you for $3,125.

The company is genuine, and so is the certification. Google's own advertising policy lists them as a required third party, which fully resolves any doubt about their legitimacy.

Whether it is actually worth the money is a different matter. A private company charges $2,150 per website each year for the right to advertise on platforms it does not own, which is essentially a toll, and tolls hit small operators the hardest. Nor is it going away, since none of the major platforms want to bear the liability of making these judgments on their own.

What it costs

Applying costs $975 per website, nonrefundable, and it is due whether you are approved or not.

Once approved, it is $2,150 per website annually, and every year thereafter.

Expedited review costs $2,500 in addition to the application fee and gets you a review that begins within two business days instead of waiting in line.

Probationary certification runs $3,995 per website per year, meant for merchants certified under closer and more frequent monitoring. This one is not your choice. It is what you are given when your application clears with reservations.

The per-website detail is the part most people overlook. Two brands means paying double.

What it does not do

Certification does not make you compliant. You were already expected to be compliant. Everything on the checklist, the state licenses, the DEA certificates, the HIPAA documentation, is something you need to have regardless.

What it provides is access. Your ads go live, your payments process without issue, and you operate openly instead of banking on nobody catching on.

That is why the certification itself is not truly the difficult part. Putting together the folder that backs it up is, and the clinics that need four months instead of two weeks are those that applied before the folder was complete.

What ClinicBuilder does, in detail

ClinicBuilder creates a telehealth clinic within your own LLC and delivers it live in roughly 31 days, with the LegitScript application submitted in week one together with the entity work and the payment processor applications.

The remaining parts of the build proceed simultaneously. Website, branding, three medication funnels ready for sale, a HIPAA compliant portal connecting doctor, pharmacy and patient with intake, e-prescriptions and messaging, a provider network spanning all fifty states, pharmacy routing, and two approved processors with routing and failover. Six workstreams, a dedicated project manager, Friday updates on Slack.

That list is not a side note to certification. It is the folder LegitScript requests. The provider network is what ensures the states listed on your website match the states on your licenses, which is the most frequent cause of an application being rejected. The pharmacy routing and the HIPAA compliant portal are two additional items a reviewer will inquire about.

The certification fees remain yours and remain unchanged. ClinicBuilder's $35,000 launch build pays for the work, not LegitScript's fee, and the LegitScript account is among the things that remain in your name, alongside the entity, the brand, the domain, the site, the funnels, the ad account, the creative, the patient list, the portal data and the processor accounts.

You never practice medicine at any point in this. ClinicBuilder's FAQ states it clearly, that you never make a decision regarding a patient, and that the provider group establishes its own protocols and has the final word every time.

While ClinicBuilder handles your ongoing monthly management, it guarantees 25 paying patients each calendar month. If you miss a month, that month's management fee is fully refunded, and the patients stay with you. Miss the first three months, and the launch fee is returned as well, and the clinic remains yours. Their wording further states that this is a marketing overview, not the contract, and it is not a promise of profit, recurring revenue, or a sale price.

In brief

LegitScript is a private business that has turned into the gatekeeper for advertising and payments in US telehealth, and it costs $3,125 per website in the first year.

There is no choice but to pay it. Paying twice, since the initial application was submitted before the documentation was ready, is a choice, and that is the only piece of this that anyone truly controls.
